Making an Appointment
Call the office (205-912-2000), and one of our staff will schedule an appointment for you. If staff are not available, you may leave a voicemail message for the doctor, who will return your call and assist you with scheduling an appointment. Staff can help you with questions about insurance, doctor specialties, and directions. Each doctor has his or her own schedule of days and hours of operation. Although the office may be closed at times during the day for telephone calls, most doctors continue to see clients during this time.

It will be helpful to you and us if you are able to download and complete these forms prior to your first session and bring them with you. If you are unable to download them, the forms can be mailed to you.

Fees & Insurance The cost to you for mental health services depends on your insurance coverage for mental health benefits and on the type of service requested. Office staff can help with your insurance & fee questions and/or you can call your insurance carrier prior to confirming an appointment. Before your appointment time you will need to know:
- Is the doctor you wish to see covered by your insurance plan?
- What is the plan for mental health coverage, including limit to number of sessions?
- Do you have a deductible and has it been met?
- What is your co-pay?
Changing or Canceling an Appointment Only our office staff or your doctor at 912-2000 can accept a cancellation or rescheduling of appointments. Although we do have a reminder call service, this cannot be used for canceling appointments. If you need to cancel an appointment please let us know as soon as possible. Appointments canceled with 24-hour notice are not charged. Cancellations with less than 24-hour notice or forgotten appointments are charged.
